Days 1–3 after IUD insertion: cramps and spotting

Days 1–3 · After-IUD guide

A day or two has passed since the procedure. The uterus is getting used to the new object; this adjustment period shows itself as cramps and spotting.

On this page you will learn to tell what is normal from what requires a call.

Short answer

How many days do cramps and spotting last after IUD insertion?

Period-like cramps and spotting are common in the first days after IUD insertion; how long they last varies from woman to woman, and SoftCare Clinic's doctor tells you based on the structure of your uterus. Spotting is expected; soaking one pad per hour is not. If such bleeding continues, call the clinic. There are no restrictions on daily life.

Course of spotting and bleeding

Spotting is most noticeable in the first days; it decreases over the following weeks.

Why do cramps happen?

The uterus is a muscle and responds to the foreign object inside it by contracting. In the first days these contractions feel like period pain, then become less frequent. In women who have not given birth, the uterus is smaller, so cramps may be somewhat more noticeable; this is one of the reasons our clinic prefers a short IUD for these women.

Which bleeding is normal?

Spotting on the pad, brown or pink discharge, and occasional light period-like bleeding are normal. Our clinic's limit is clear: if you are soaking one pad per hour and this continues, call. What matters is not a single episode of somewhat heavier bleeding, but its continuing.

What is allowed, and what is not?

  • Bath, pool, sea: no restriction.
  • Sport and heavy lifting: no restriction.
  • Sex: no restriction; if spotting bothers you, you can wait.

Do

  • Keep an eye on the color and amount of spotting for a day or two; it should be decreasing.
  • For cramps you can use a warm shower or a hot water bottle.

Don't

  • Don't take medication on your own to reduce the bleeding.
  • Don't panic and interpret pain as "the IUD has slipped"; slipping is recognized by feeling an object in the vagina.

Special note

Pad or tampon?

In the first days a pad is more practical for monitoring spotting; you can see how much you are bleeding. Tampons are not forbidden.

When should you call?

Call the clinic right away

  • Heavy bleeding: Soaking one pad per hour, and this continuing.
  • Fever and foul-smelling discharge: Fever, weakness with fever, or foul-smelling discharge.

Call the clinic

  • If instead of spotting you have bleeding that soaks one pad per hour and it continues.
  • If fever or foul-smelling discharge has started.
  • You feel a hard tip: If you feel a hard object or the tip of the IUD in your vagina.
  • Object in the vagina: If you notice that the IUD has come out of place or there is an object inside your vagina.
  • Pain that does not go away: Abdominal pain that lasts beyond the first days and keeps getting worse.
