2–3 months after IUD insertion: the bleeding pattern settles

Months 2–3 · After-IUD guide

Two to three months have passed. This is the end of the "adjustment period" for the IUD: spotting has mostly stopped, and periods have come close to their own pattern.

This page explains what counts as settled and what your options are if it has not settled.

Short answer

How many months after IUD insertion do periods return to normal?

According to SoftCare Clinic's doctor, the bleeding and spotting pattern settles 2–3 months after IUD insertion. After this, periods in most women are close to their previous pattern. If the pattern has not settled or bleeding affects your quality of life, talk to the doctor; there is no time rule for removing the IUD, it can be removed whenever you wish.

What does "settled" mean?

  • Spotting between periods has stopped or is very infrequent.
  • Period length and pad count are similar two months in a row.
  • Cramps are limited to the period and respond to painkillers.

If the pattern has not settled

If spotting between periods continues, you soak one pad per hour every month during your period, or pain disrupts daily life, talk to the doctor. The position is reassessed with ultrasound; sometimes a cause related to size or position is found. According to our clinic's doctor, there is no time rule for removing the IUD: it can be removed at an examination whenever you wish, and fertility returns immediately.

From now on

Annual gynecological examinations and smear/HPV follow-up continue; the IUD does not change these screenings. The recommended duration of use is 5 years depending on the model, or the duration stated by the manufacturer; it can be replaced before this time is up.

Do

  • At the end of three months, share your period notes with the doctor.
  • Plan your annual check-up and smear dates.

Don't

  • Don't try to pull out the IUD yourself because the pattern has not settled.

Special note

If the IUD is removed

Removal is shorter than insertion: the doctor sees the strings with a speculum and pulls gently. A new IUD can be inserted right away, or you can switch to another method; the decision is yours and your doctor's.
