Weeks 3–4 after IUD insertion: as spotting decreases
Weeks 3–4 · After-IUD guide
The check-up is behind you, and the position has been confirmed. In these weeks many women ask, "I'm fine, but why hasn't the spotting stopped?"
The answer lies in the IUD containing no hormones: it slowly leaves the uterine lining to its own rhythm, and this is completed over a few months.
Short answer
Is it normal to still have spotting one month after IUD insertion?
It can be. With a copper and gold IUD, spotting between periods may continue in the first months; according to SoftCare Clinic's doctor, the bleeding pattern settles in 2–3 months. If your check-up has been done and the position confirmed, spotting alone is not a problem. Call if you have bleeding that soaks one pad per hour, fever or a foul smell.
Course of spotting and bleeding
Spotting has decreased but may not have stopped completely; the first period is approaching.
Bleeding curve with day 24 marked; spotting at a low level, the first period window approaching.Dashed line: day 15 check-up. Shaded areas: periods. Representative; durations vary from person to person.

Why does spotting continue?
Copper ions create a mild foreign-body reaction inside the uterus; this is part of how the IUD prevents pregnancy. The same reaction is felt in the first months as spotting between periods and somewhat heavier periods. The gold is there to slow the wear of the copper wire; it does not promise less spotting.
As the first period approaches
The IUD does not stop ovulation; your cycle follows its own schedule. The first period comes when you expect it, perhaps a few days earlier or later. It may be heavier than usual; the next page explains this in detail.
Protection in this period
Protection has continued since the day it was inserted. If your period is late or you suspect pregnancy, our clinic follows up with a blood hCG test; don't conclude on your own that the IUD has failed.
Do
- Note the days of spotting; these notes will be useful at your first period.
- Eat iron-rich foods; the first periods can be heavy.
Don't
- Don't take hormones or medication to stop bleeding on your own decision because of spotting.
Special note
Breastfeeding mothers
While breastfeeding, the cycle may already be irregular; a delay in the first period may be due to breastfeeding rather than the IUD. Your doctor explains additional points for breastfeeding women and women who have not given birth at the examination.

Frequently asked questions
Can my period be late after IUD insertion?
Because a copper IUD does not affect ovulation, your period usually comes on time. If it is late, SoftCare Clinic rules out pregnancy with a blood hCG test; a lasting delay due to the IUD is not expected.
Can I get pregnant while protected by an IUD?
It is very rare. According to our clinic's doctor, protection starts the moment it is inserted; in case of doubt, follow-up is done with an hCG test, and the doctor decides on any additional method.
When does the spotting stop?
According to SoftCare Clinic, the bleeding pattern settles in 2–3 months. Spotting may continue while decreasing; if it is increasing or soaking one pad per hour, call the clinic.
